What Are Dental Inlays and Onlays?

Dental inlays and onlays are indirect dental restorations custom-made in a dental laboratory to fit the prepared cavity or tooth precisely. They provide an ideal solution for moderate tooth damage: more extensive than what fillings can fix, but less severe than what requires a full crown.

Inlays

Inlays fit entirely within the cusps (the raised points) of a tooth’s chewing surface, much like a puzzle piece. They restore the biting area without covering the cusps themselves. This option preserves more natural enamel and tooth structure compared to full crowns, making inlays a conservative yet strong restoration.

Key features of inlays:

  • Crafted from materials such as porcelain, composite resin, or gold alloys.
  • Porcelain and composite resin inlays are favored for their natural tooth-like appearance and resistance to staining, making them excellent for visible teeth.
  • Gold inlays offer exceptional durability and are often used for molars, which endure heavy chewing forces.

Fabrication and bonding:

  • Inlays are fabricated outside the mouth using digital impressions or traditional molds, ensuring superior precision.
  • This indirect process provides a precise fit and strong bond to the tooth, minimizing gaps that bacteria and plaque could accumulate in, reducing the risk of future tooth decay and gum inflammation.

Onlays

Onlays, sometimes called partial crowns, extend the coverage beyond inlays by restoring one or more cusps of the tooth as well as the biting surface. They cover a larger area, making them suitable for teeth with more extensive damage or cracked cusps.

Key features of onlays:

  • Made from durable materials: porcelain, composite resin, or gold alloys; materials that balance aesthetics and strength.
  • Porcelain onlays mimic the translucency and color of natural teeth and resist wear and staining.
  • Composite resin onlays are cost-effective with decent esthetics but may have a shorter lifespan.
  • Gold onlays provide superior strength for posterior teeth under heavy load.

Advantages in preservation and bonding:

  • Onlays preserve more of the tooth structure than full crowns, reducing the need for aggressive tooth reduction, which can weaken teeth or cause root canal issues.
  • Dentists bond onlays into place after precise fabrication using digital or traditional impressions, ensuring a perfect fit that protects remaining tooth tissue and restores chewing function.

Technological advancements:

  • Computer-aided design/computer-aided manufacturing (CAD/CAM) technology allows efficient and highly accurate onlay production, often reducing patient visits and enhancing comfort.
  • Clinical studies confirm that well-designed and bonded onlays preserve natural tooth structure while providing strength and clinical success comparable to crowns, supporting their usage in modern conservative restorative dentistry.

Why Are Dental Inlays and Onlays Important?

Inlays and onlays play a critical role in dental health by:

  • Preserving Natural Tooth Structure: They require less removal of healthy tooth tissue than crowns, maintaining more of your natural enamel and dentin.
  • Preventing Further Damage: By reinforcing weakened teeth, they reduce the risk of cracking or fracture that could necessitate root canals or extractions.
  • Enhancing Esthetics: Custom shading ensures nearly invisible repairs, matching your natural teeth.
  • Improving Chewing Function: Inlays/onlays restore proper tooth shape and bite alignment for comfortable eating and speaking.
  • Long-Term Cost-Effectiveness: Their durability reduces the need for repairs and replacements over time.

Choosing inlays or onlays over large fillings or crowns often results in healthier teeth, better appearance, and fewer future complications, aligning with modern conservative dentistry principles.

Who Needs Dental Inlays or Onlays?

You may benefit from dental inlays or onlays if:

  • You have cavities or tooth damage that is too extensive or fragile for traditional fillings.
  • You have cracked, fractured, or worn cusps in molars or premolars.
  • You want a more durable, long-lasting restoration option.
  • You seek tooth-colored restoration for a natural smile.
  • You need to replace old, damaged fillings with stronger materials.
  • You prefer to preserve as much of your natural tooth as possible.

Early evaluation can help catch damage before it escalates to the need for crowns or extractions. Schedule an exam with Dr. Lieneck for personalized recommendations.

Benefits of Dental Inlays and Onlays

Precision Fit: Each restoration is meticulously crafted to fit your tooth’s unique anatomy, preventing food trapping and reducing future decay risk.

Strength & Durability: Porcelain and composite inlays/onlays endure biting forces effectively and resist wear, lasting many years with proper care.

Natural Appearance: Custom color matching and translucency create restorations indistinguishable from natural teeth.

Conservative Treatment: These restorations conserve healthy tooth tissue, preserving tooth vitality and reducing sensitivity.

Resistance to Stains: Unlike composite fillings, porcelain inlays/onlays maintain their beautiful appearance over time.

Comfort & Functionality: Smooth surfaces and exact fit restore proper bite and chewing comfort.

Advancements in CAD/CAM technology and material science have made these restorations safer, faster, and more patient-friendly than ever before.

The Dental Inlays and Onlays Process at Sunflower Dental Spa

1. Initial Consultation and Evaluation

Dr. Lieneck performs a detailed oral exam and diagnostic imaging to assess tooth condition and candidacy for inlays/onlays. We discuss your goals and treatment plan in depth.

2. Tooth Preparation

Decay or old fillings are removed, and the tooth is shaped to provide a stable base for the restoration while preserving as much natural structure as possible.

3. Impression or Digital Scan

We capture the precise tooth shape using digital intraoral scanning or traditional molding materials for custom lab fabrication.

4. Temporary Restoration Placement

A temporary filling protects the prepared tooth while your custom inlay or onlay is being made, usually within 1-2 weeks.

5. Bonding Your Restoration

At your next visit, we remove the temporary and bond the custom restoration using strong, biocompatible adhesives. Dr. Lieneck ensures perfect fit and comfort through bite adjustments.

6. Final Polishing and Care Instruction

The restoration is polished to a smooth finish, and you receive guidance on caring for your new inlay/onlay to maximize its lifespan.

Our modern equipment and gentle approach prioritize your comfort throughout the process.

What to Expect After Receiving Inlays or Onlays

You may experience mild sensitivity to temperature or pressure during the first few days, which resolves as the enamel adjusts. Maintain meticulous oral hygiene by brushing gently and flossing around restorations.

Avoid chewing very hard or sticky foods during the initial recovery period to prevent damage. Attend regular dental check-ups at Sunflower Dental Spa to monitor restoration integrity.

FAQ's

Are dental inlays and onlays painful?

The procedure for dental inlays and onlays is done under local anesthesia, which numbs the area completely, ensuring you don’t feel any pain during treatment. After the anesthesia wears off, you may experience mild sensitivity or slight discomfort, which is normal as your tooth adjusts to the new restoration. This sensitivity subsides within a few days. To manage discomfort, over-the-counter pain relievers are usually effective. If pain persists or worsens, contact your dentist promptly.

How long do dental inlays and onlays last?

With proper oral hygiene and regular dental check-ups, dental inlays and onlays can last anywhere from 10 to 30 years. The lifespan depends on factors like the material used (porcelain generally lasts longer), the location of the restoration (molars endure more stress), and how well you care for your teeth. Good habits such as brushing, flossing, and avoiding habits like nail-biting or chewing ice can significantly extend the life of your restoration.

Can I eat normally after treatment?

It’s advisable to avoid hard, sticky, or crunchy foods for the first 24 to 48 hours after receiving dental inlays or onlays to allow the bonding to set and the tooth to adjust. After this period, you can generally resume your normal diet with normal chewing habits. However, be cautious with extremely hard foods initially, and avoid habits like chewing ice or using your teeth as tools to protect your restoration and nearby teeth.

Are inlays and onlays covered by insurance?

Many dental insurance plans partially or fully cover the cost of inlays and onlays, especially when they are used to repair damage beyond the extent of a simple filling. It’s important to check with your insurance provider about your specific coverage and benefits. At Sunflower Dental Spa, we assist patients with insurance verification and can provide detailed cost estimates to help you understand your out-of-pocket expenses before treatment.

How do I clean my inlay or onlay?

Maintain good oral hygiene by brushing twice daily and flossing carefully. Regular professional cleanings are important to keep restorations healthy.

What is the difference between crowns and inlays/onlays?

Crowns cover the entire tooth, while inlays/onlays conserve more natural tooth and repair only damaged parts. While crowns are often necessary for more extensive damage, inlays and onlays are ideal for moderate repairs. For a comparison of different crown types and their costs, visit our Dental Crowns: Types & Costs page to help you make an informed decision about your restorative needs.

Seriously the most kind, caring, thorough dentist I have ever been to. Dr. Wetmore was kind and attentive to my anxiety while at the same time making sure I was comfortable through a long drilling for a crown and very thorough making sure I was numb enough and the bite would be just right. I struggle with self care and this is the first dentist in my life where I wasn't shamed for it. And even though I was scared for the procedure I felt so safe here. Awesome place! The front desk staff, hygienist I saw and assistants were all super skilled and amazing too. Thanks ladies!

Sunflower Dental Spa is for me the FIRST Dental Office that I enjoy going to. I walk in and I am greeted by, 3 Wonderful Ladies with Beautiful Smiles. And even though they know that I can never remember their names, they know mine.
Rachel, my Dental Hygienist, took me back to begin my cleaning... Well at my last cleaning, I misunderstood something that she told me... so this time I asked her to please show me... and she did.
Once Rachel finished, she went and told my Wonderful and Most Excellent Dentist, Dr. Justin Lieneck that it was his turn to come in and check out my teeth. As Justin is checking out my teeth, I asked him, "Do the look okay? You are taking an awfully long time..."
Justin said, "I'm checking each one of them...but I don't see cavities." That isn't exactly what he said, but I can't remember word for word, like I used too.
But if you are looking for a place to go that will take care of your Teeth.... I Highly recommend Sunflower Dental Spa 🌻
Excellent team and dental care .

It’s hard to put my appreciation into words. Dr. Wetmore and her staff were so helpful and welcoming. They were able to work with my insurance and I to go from consultation to surgery in less than a month, and for half the price I was quoted from Aspen. I was very nervous going into the surgery and amount of extractions I needed but was put at ease with all of the commendations they offered. I highly recommended the laughing gas for anyone that feels the same way. The entire surgery was painless, completely painless and took about two hours. It’s now one day post op and other than some soreness I have never felt better. Sunflower dental deserves all the praise they get in these reviews. Truly an amazing experience. I am so grateful.
